If you plan to purchase from us on a regular basis and would like to establish an account to "pay as you go", please follow these simple steps

1. Print out a copy of our COD Application in PDF format. (If you don't have Adobe Acrobat to read PDF files you can get it here: Get Acrobat Reader
2. Complete and sign the application(s).
3. Fax the signed application(s) to:
